Intensive Case Management (ICM)

Description

Intensive Case Management (ICM) is a program intended for people with severe or complex mental disorders.

The ICM program offers psychosocial rehabilitation and assistance to people directly in their living environment and community. ICM specifically helps them develop functional, personal and interpersonal skills to increase their independence and to continue their recovery process.

The ICM interdisciplinary team is composed of diverse professionals such as nurses, social workers, special educators, psychoeducators and occupational therapists. There are around 100 ICM teams in Québec.

The frequency of the contacts is adapted to each person’s needs.

This program is part of a range of mental health care and services offered in the health and social services system.

Clients

The ICM is designed mainly for people who:

  • are adults or transitioning to adulthood
  • present with a severe mental disorder, diagnosed mainly with a psychotic disorder, a mood disorder or an anxiety disorder
  • have needs requiring psychosocial rehabilitation services in their living environment
  • wish to return to live in independent housing
  • have co-occurring mental health and substance use disorders.

Services offered

ICM teams offer three types of services:

  • Psychosocial rehabilitation. This is offered directly in the person’s living environment.
  • Case management. This involves putting service users in contact with the different resources required for their re-entry into the community, including family members and relatives.
  • Outreach. This consists in proactively approaching people wherever they are with a view to building bonds of trust, carrying out intervention strategies and accompanying them to the most appropriate services and resources.

Rehabilitation activities offered

ICM professionals offer services that support the recovery process through activities related to the following needs:

  • Attending appointments and participating in significant activities
  • Taking medication and recognizing the early warning signs of a relapse
  • Ability to hold onto housing (daily living and housekeeping skills, financial management, etc.)
  • Problems with substance abuse or with the law
  • Resolution of psychosocial crises and situations presenting a danger to self or others
  • Developing a life plan requiring psychosocial rehabilitation
  • Assistance in developing a  meaningful and supportive social network

Access to the program

To access the ICM program, you do not need a medical referral. You can talk about it with your doctor or a psychosocial worker. You can also contact the psychosocial reception desk at your CLSC or call Info-Social 811. They will put you in touch with the people who can respond to your request.
