Procedure for requesting personal information
Anyone may request his or her personal information from the Québec Vaccination Registry.
In order to prevent identity theft and to protect your personal information, your request must be accompanied by a copy of your health insurance card or birth certificate.
For minors under 14 years of age, photocopies of a parent, representative or legal guardian’s valid I.D. document and that of the minor (health insurance card or birth certificate) are required.
Persons 14 years and over must complete and sign their own requests.
Québec residents
To submit a request, follow these steps:
- Complete and sign the Access to Information on Vaccines Request form

- Send the form, along with a photocopy of a valid I.D. document (health insurance card or birth certificate) to your regional officer in charge of requests, by one of these ways:
- Post mail
- Fax
You may contact your integrated health and social services centre (CISSS) or your integrated university health and social services centre (CIUSSS) to receive a printed version of the form.
Access to information on vaccines requests, and responses to them, will not be dealt with by telephone.
If you discover that the information is incomplete, you may request access to information from each establishment that administered vaccines.

Residents outside Québec

If you do not have aRégie d’assurance maladie du Québec (RAMQ) health insurance number, you do not need to specify one. - Send the form, along with a photocopy of a valid I.D. document (health insurance card or birth certificate) to your regional officer, by one of these ways:

Once your request has been received, the officer in charge of access and protection of personal information will send your request by mail. The response time is about 2 weeks following receipt of the request. This response time may be longer depending on the number of requests received.
Access to information on vaccines requests, and responses to them, will not be dealt with by telephone or in person.
Acknowledgements of receipt will not be sent for faxed or mailed requests.
If you discover that the information is incomplete, you may request access to information from each establishment that administered vaccines.
