This optional evaluation is intended for people who have used the oncology passport. By giving your opinion, you will allow the Ministère de la Santé et des Services sociaux to improve its services for patients.
In order to preserve your anonymity, do not provide personal information that identifies you (for example, your name or your employer) or other people.
Only authorized persons will have access to the answers. The information collected will be kept on secure servers of the Ministère de la Santé et des Services sociaux for one year, after which it will be destroyed.
