Complaint regarding food safety or food poisoning

Don’t hesitate to submit a complaint if:

• You believe that a food establishment is not following health and safety regulations.
• You have found a foreign body in a food item.
• You felt ill after eating a food item in a restaurant.

Some complaints may lead to an inspection and, if consumer protection is involved, to penalties.

In order for your complaint or report to be admissible, you must have witnessed the situation yourself and be able to say exactly where it occurred. This form can only be used to file a complaint against a food establishment located in Québec. Furthermore, the complaint must not concern the subject of litigation between the complainant and the operator in question. Finally, it must be filed within the timelines specified in the form.
