Your integrated health and social services centre (CISSS) or your integrated university health and social services centre (CIUSSS) may contact you or send you promotional material on vaccines. They will also be able to contact you if you must receive other doses of a vaccine.
The vaccines you should get are based on the regular vaccination schedule and the vaccines you have already received.
Refusal to receive promotional material or vaccination reminders
At any time, you may refuse to have a CISSS or CIUSSS send you:

Your refusal can be permanent or temporary. You can modify your decision at any time.
To express your refusal
You can express your refusal by completing the Refusal form or modification of refusal form . Once completed, mail it to the address indicated on the form. Make sure that you provide your health insurance number. You can contact your CISSS or CIUSSS to obtain a paper version of the form.
To cancel your refusal
