14-Day Notice – Proof of firearm registration

Complete this form if a wildlife protection officer issued you a 14-Day Notice for failing to present proof of firearm registration as required under the Act respecting the conservation and development of wildlife (C-61.1) and the Firearms Registration Act (I-0.01).

The information is collected by the Ministère de l’Environnement, de la Lutte contre les changements climatiques, de la Faune et des Parcs.

Once your form has been submitted, a confirmation number will be displayed. We recommend that you take note of it.

Choose one of the three options to send us your proof of firearm registration.

Option 1: If you are in possession of the firearm registration

Attach a photocopy or photo of the original firearm registration certificate. 

Option 2: If your firearm registration application was submitted by mail

 

Option 3: If your firearm registration application was submitted online via the Service d’immatriculation des armes à feu website

Visit the Service d’immatriculation des armes à feu portal to find your application number.

Confirmation and submission

Pursuant to the Act respecting the conservation and development of wildlife (C-61.1) and the Firearms Registration Act (I-0.01), I declare that I have submitted a firearm registration application for the firearm identified in Part B of Form PFQ-597, received from a Québec wildlife protection officer. I declare that I am the owner of the said firearm and hereby provide proof by completing this form within the prescribed time frame.

  • I declare that all information provided in this form and in all attached documents is complete and accurate.

Only authorized individuals will have access to the collected information, and only as part of their official duties. This information will be retained for 12 months, after which it will be destroyed.
