Seven-day Notice – Not carrying the required licence

Complete this form if a wildlife protection officer or assistant issued you a 7-day notice for failing to present a required permit, certificate, or authorization under the Act respecting the conservation and development of wildlife.

The information is collected by the Ministère de l’Environnement, de la Lutte contre les changements climatiques, de la Faune et des Parcs.

Once your form has been submitted, a confirmation number will be displayed. We recommend that you take note of it.

Confirmation and submission

Pursuant to the Act respecting the conservation and development of wildlife (C-61.1), I declare that I have the required permit, certificate, authorization, lease, and/or photo identification and I submit the proof indicated in Part C of Form PFQ 596, received from a Québec wildlife protection officer, within the prescribed time frame.

  • I understand that refusing to provide consent may result in a fine;  
  • I declare that all information provided in this form and in all attached documents is complete and accurate.

Only authorized individuals will have access to the collected information, and only as part of their official duties. This information will be retained for 12 months, after which it will be destroyed.
