Last-Resort Financial Assistance Application Forms
To apply for last-resort financial assistance under the Social Assistance Program or the Social Solidarity Program, you must fill out the forms required depending on your situation.

Simplified application for financial assistance for a person with a claim slip
If your situation changes while you are not receiving financial assistance, but you have a claim slip (drug card), you may submit a simplified application for financial assistance.
To apply, you must fill out the form below and submit it in person at a Québec Services office, by mail or online using My Account.

First application for last-resort financial assistance for refugee protection claimants
If you are a refugee protection claimant, there are two ways to submit your first application for last-resort financial assistance.

You can also apply in person at a Services Québec office or by mail using the First Application for Last-Resort Financial Assistance – Refugee Protection Claimants form (3002A) (PDF 1.17 Mb). This form is for refugee protection claimants who are applying for the first time.
