Obtaining a Special Benefit for Medical Transportation or Therapeutic Activities Under a Social Assistance Program

If you receive benefits under a social assistance program, you may be eligible for reimbursement of medical transportation expenses or expenses related to participation in therapeutic activities recommended by your physician.

The social assistance programs are as follows:

• Social Assistance Program
• Social Solidarity Program
• Aim for Employment Program
• Basic Income Program

To get reimbursed for your medical transportation expenses, you must provide one of the following documents:

• Written confirmation of the medical visit from the institution’s staff
• The Application for Reimbursement of Transportation Expenses – Medical Visits or Therapeutic Activities form (SR-2589A) (PDF 287 Kb) and the required supporting documents.

Depending on the means of transport you use, other information may be required, such as the means of transport, distance in kilometres, invoices or receipts, etc.

In some situations, a certificate justifying your need for transport may be required. If this is the case, the Ministère will inform you which documents to submit.

You may be eligible for reimbursement of transportation expenses when you participate in therapeutic activities recommended by your physician.

To apply, you must submit your physician’s medical recommendation.

You must also submit one of the following documents:

• Proof of participation or a certificate of attendance for your therapeutic activities
• The Application for Reimbursement of Transportation Expenses – Medical Visits or Therapeutic Activities form (SR-2589A) (PDF 287 Kb)
• The Application for Reimbursement of Transportation Expenses – Therapeutic Activities: Public transit form (SR-2590A) (PDF 259 Kb)

Depending on the means of transportation you use, other information may be required, such as the means of transportation, distance in kilometres, invoices or receipts, etc.

You must submit the supporting documents no later than 90 days after the date of your trip. If you make several trips during the same month, the 90-day period starts on the date of the last trip in the month.

You can submit your documents in person or by mail to your Services Québec office.
