You must complete this form to submit your application for permanent selection.

Your spouse or de facto spouse accompanying you to Québec must also complete and sign this form.

Contrat d’autonomie financière – Travailleurs qualifiés (Financial Self-Sufficiency Contract — Skilled Workers) (PDF 297 Kb) (in French only)

You must complete this contract, by which you agree to provide for your needs and those of the members of your family accompanying you for at least the first three months following your arrival in Québec.

Declaration of the candidate for immigration (PDF 562 Kb)

You must complete this form (available in French only) if you submitted an application for permanent selection online and you are using the services of a representative in your procedures.

Declaration of a candidate practising the teaching profession in preschool, elementary or secondary school (PDF 514 Kb)

You must complete this form if you or your spouse practise or have the training to practise one of the following professions:

• Preschool, elementary or secondary school instructor
• Elementary or secondary school teacher

Déclaration des conjointes ou conjoints (Declaration by spouses) (in French only) (PDF 287 Kb)

You must complete this declaration if your spouse or de facto spouse is accompanying you to Québec. Both of you must sign this declaration.

Declaration of consent - Permanent immigration of a minor child accompanied by only one parent (PDF 129 Kb)

If you wish to immigrate with your dependent child(ren) under the age of 18 without the other parent accompanying you, you must:

• Submit the form "Declaration of consent - Immigration of a minor child accompanied by only one parent ME-0045-2402 (PDF 129 Kb)"  as part of your application for permanent selection.
• Have this form completed and signed by the non-accompanying parent;
• Include proof of identity of non-accompanying parent.

Declaration by a physician trained outside Canada and the United States (PDF 553 Kb)

You must complete this form if you or your spouse practise or have the training to practise one of the following professions:

• Medical specialist
• General practitioner
• Family physician

Declaration by a candidate practising a regulated trade or profession (PDF 553 Kb)

You must complete this form if you or your spouse practise or have the training to practise a regulated profession or trade in Québec. Take note of the conditions for admission to the professional order before you apply.

Declaration on the democratic values and the Québec values (PDF 313 Kb)

You must complete this form (available in French only) and agree to:

• Learn the French language, if you don’t already speak it;
• Know and respect the democratic values and the Québec values.
