Register of Origin Information in Procreation Involving a Third Person
The register of origin information in procreation involving a third person allows for any person born of a procreation project involving a gamete donation or a surrogate pregnancy to obtain information about the person who contributed to the procreation, if it is available.
Created on June 6, 2025, it allows various stakeholders such as assisted reproduction clinics, courthouse registries, the Ministère de la Santé et des Services sociaux and the Directeur de l’état civil to register or transmit information or to file documents in their possession. The register will be fully established using the information and documents available from the various stakeholders on June 6, 2026.
Register Content
If your birth was the result of a gamete donation or a surrogate pregnancy, you may use the register to obtain information about the person who contributed to the procreation and documents related to the process of knowing one’s origins.
Thus, you can know
their first and last name
their age
their ethnic origin
their civil status
their level of education, diplomas and field of study, if any
their profession, if any
their height
their skin colour
their eye colour
their hair colour and texture
their personality traits, special skills, preferences and hobbies
The register also contains documents regulating surrogacy projects involving surrogate mothers domiciled in Québec or outside Québec. For parental projects involving a gamete donation (egg cell or sperm donation), the register does not contain any documents.
Contact information of the person who made the donation or the surrogate mother
You will also have access to that person’s contact information, if they agree.
If your birth was the result of a surrogacy pregnancy, you may obtain certain documents, including a copy of the surrogacy agreement.
If a person donated sperm or eggs before June 6, 2025, their identity will remain confidential. However, the person can agree to have this information disclosed to you.
If a person donates sperm or eggs on or after June 6, 2025, their identity will be accessible. However, the person can refuse to have their contact information disclosed, for donations made before June 6, 2025, as well as for donations made after. This refusal will apply to all donations.
Persons Authorized to Request Information and Documents
Requests for information and documents may be made by persons born of a gamete donation or surrogate pregnancy. Persons aged 14 or younger may apply with the consent of their father and mother, parents, or tutor.
In the event of the death of a person born of a gamete donation or a surrogate pregnancy, their first-degree descendants aged 14 or older are entitled to make a request.
A person whose birth was the result of a procreation involving a third person or their first-degree descendants, if deceased, may obtain the information and documents contained in the register.
Physicians may also request information about the person who contributed to procreation.
If you are under 14 years of age and you are born of a procreation involving a third person, your parent(s) or tutor must complete the Appendix (PDF 647 Kb) and submit it with the request for information or documents form
If your request does not meet the criteria or if the register does not contain information concerning you, you will receive a response mentioning this.
The various parties have until June 6, 2026 to transmit or record the required information or documents in the register. Therefore, information concerning you could only be transmitted to you after June 6, 2026.
When you fill out the form, you can indicate that you wish to obtain a certificate to have access to the documents contained in the court file by checking the corresponding box.
This certificate confirms that your birth was the result of a procreation involving a third person, or that you are a first-degree descendant of such person, if deceased, and confirms whether you can obtain contact information for the third person who contributed to the procreation. The certificate will indicate to the court clerk whether any information contained in the requested documents should be redacted.

You donated sperm or eggs You will be notified the first time the register receives a request for information, and will be asked whether you would like your contact information to be released. You can make any contact subject to conditions, for example by limiting it to email exchanges only. You can change your consent at all times using the consent form (PDF 673 Kb).
Donation made before June 6, 2025 If you donated sperm or eggs before June 6, 2025, your identity will remain confidential. You can, however, agree to its disclosure by providing consent using the consent form (PDF 673 Kb).
If you agreed to have your contact information disclosed to a person born of a gamete donation or a surrogate pregnancy or their descendants, if deceased, and you need to update this information, you can contact Services Québec to do so, if you wish. Otherwise, if a new request is made concerning you, we will contact the Régie de l’assurance maladie du Québec to obtain your updated contact information.
Psychosocial Support
Psychosocial support is available to anyone who makes a request for information or documents to which they are entitled. If you wish to receive support offered by the Ministère de la Santé et des Services sociaux, you can indicate this by checking the corresponding box on the form used to make a request for origin information.
These services may also be offered to anyone who has contributed to the procreation of the person who makes a request for origin information. If you wish to receive support offered by the Ministère de la Santé et des Services sociaux, you can indicate this by checking the corresponding box on the consent form. This consent form will be sent to you when an initial request for information concerning you is received. You will then be asked whether you would like your contact information to be disclosed. You can make any contact subject to conditions, for example by limiting it to email exchanges only.
Entering Information in the Register
Different departments and agencies of the Gouvernement du Québec enter information in the register. In addition, assisted procreation clinics provide information for entry.
It is not possible for an individual to enter information or submit documents to the register. However, it is possible for an individual to provide consent and request that contact information be updated when information concerning them already appears in the register.
