If you are a recipient of the Social Assistance Program, the Social Solidarity Program or the Aim for Employment Program, you can obtain payment of your living expenses for treatment in a centre offering addiction services.

At the beginning of your stay, the counsellor and the coordinator of the centre where you will receive treatment will have you sign the form. You will then have to fill out the formulaire de début de séjour pour des services en toxicomanie avec hébergement (2561A) (PDF 93 Kb).
Step form for addiction services with accommodation
The intervener and the coordinator of the centre where you are undergoing treatment will have you sign this form to inform us of the revision of your treatment plan. You will then have to fill out the formulaire d'étape pour des services en toxicomanie avec hébergement (2562A) (PDF 77 Kb).
End-of-Stay Form for Addiction Services with Accommodation
Once your stay for addiction services is over, the intervener fills out this form to inform us of the end of the intervention plan and employment integration opportunities. You will then have to fill out the formulaire fin de séjour pour des services en toxicomanie avec hébergement (2563) (French only) (PDF 77 Kb).
