Medical Transportation: Application for Payment
You can get reimbursement for medical transportation costs. For this, a written medical certificate may be required, as well as other information, such as written confirmation of a medical examination obtained from the facility's staff, mode of travel, mileage, etc.
For certain medical transportation, you must send the Application for Payment of Medical Transportation (SR-2589A) (PDF 369 Kb) form and the required supporting documents by delivering them to a Services Québec office or using your online file no later than the last day of the month following the month in which the trip was made. For example, if your request concerns a trip made on February 22, you will have until March 31 to submit your request.
Medical Transportation - Therapeutic Activities: Application for Payment
You can be reimbursed for transportation costs when you participate in therapeutic activities.
You must then send the form Application for Payment of Medical Transportation – Therapeutic Activities (SR-2590A) (PDF 346 Kb) to your Services Québec office no later than the last day of the month following the month in which the trip was made.
