Requesting an official document

You can request an attestation of the validity of a teaching licence, a certified copy of a teaching licence or a certified copy of a notice of conditional eligibility.

Attestation of the validity of a teaching licence

The attestation of the validity of a teaching licence specifies the type of teaching licence that was issued and its period of validity. To obtain this document, you must complete the form entitled Request for an attestation of good standing with regard to the validity of a teaching licence (PDF 67 Kb).

Certified copy of a teaching licence

If your teaching licence has been lost, you can request a certified copy by completing the form entitled Request for a certified copy of a teaching licence (PDF 50 Kb).

Certified copy of a notice of conditional eligibility

If your notice of conditional eligibility has been lost, you can request a certified copy by completing the form entitled Request for a new notice of conditional eligibility (PDF 100 Kb).

Additional information

Two types of copies may be requested during the application process for a teaching licence: a certified copy and a sworn copy.

A certified copy is a copy made from original documents and bears the original signature of a person acting as a legal representative of the organization that issued the document.

A sworn copy is a copy made from original documents in respect of which the holder has taken an oath before a Commissioner for Oaths and has declared that the copy is a true copy of the original document. To find a Commissioner for Oaths, consult the Register of Commissioners for Oaths.

In both cases, the person who acts as a legal representative or as a Commissioner for Oaths must indicate on the copy their name, title or functions, their contact information in block letters, as well as their address and the telephone number where the Ministère can reach them.
