Complaint or comment about a service, program or measure received

A complaint or comment may be made to the department regarding the accessibility or quality of a service, program or measure.

Power of attorney

You can be represented by someone you designate to file your complaint by using a power of attorney. The department prefers that you complete the Power of Attorney form (PDF 237 Kb). You can also write a letter stating the name of the person you are authorizing to act on your behalf.

You can submit your complaint in four ways:

Complaint about the management of a contract

The department offers a mechanism for handling complaints about the management of a contract. Complaints must relate to a contract award process.
