Forms for tutors and the tutorship council

Annual Management Report – Minor (fillable) (PDF 1.30 Mb)
This form is used to submit the annual management report for the minor’s patrimony.

Attestation of a Tutorship Council Decision Regarding the security Required from a Tutor (PDF 1.95 Mb)
This form is used to record and communicate the tutorship council’s decision to the tutor regarding the security to be provided.

Confirmation of Hold Funds (PDF 1.86 Mb)
This form must be filled out by the financial institution to confirm the hold funds requested.

Exceptional Use of the Patrimony of a Minor (fillable) (PDF 635 Kb)
This form is used to request the use of the minor’s patrimony in exceptional circumstances.

Final Administration Report – Minor (fillable) (PDF 1.83 Mb)
This form is used to submit the administration report for the period covering the final annual report and the end of your role as tutor.

Inventory by Private Writing (before two witnesses) (fillable) (PDF 1.49 Mb)
This form is used to produce a list of all possessions belonging to the minor worth $100 or more, as well as their debts.

Request to a Financial Institution Regarding the Holding of Funds (PDF 1.19 Mb)
This form is used to request a hold funds as a security from the financial institution where the minor’s funds are held.

Miscellaneous forms

Declaration of the Remittance of Property in Favour of a Minor (PDF 49 Kb)
This form can be used by anyone to notify the Curateur public of the remittance of property to a minor 15 days prior to doing so. This allows the Curateur public to inform the tutors of their obligations.

Declaration of Simple Emancipation (PDF 696 Kb)
This form allows the tutor, with support from the tutorship council, to inform the Curateur public of their declaration authorizing the simple emancipation of a minor.

Declaration of Dative Tutorship (PDF 417 Kb)
This form allows a parent to designate, in advance, the person who will care for their child should they die or become incapable.
