Complete a declaration

Your parents, sponsor or spouse must first create or log in to their online file to report their income.

They must then provide the following information:

• their contact information (address, telephone, etc.)
• their social insurance number
• information allowing us to link you to them (first name, last name and date of birth as well as your permanent code)
• the amount shown on line 199 of their last Québec income declaration (tax return)

Your parents, spouse and sponsor can complete a declaration online.

The forms to be filled in to report their income are the following:  

• 1150 - Declaration of Father or Sponsor (PDF 133 Kb)
• 1151 - Declaration of Mother or Sponsor (PDF 132 Kb)
• 1152 - Declaration of Spouse (PDF 89 Kb)

A person who contributes financially to your studies may want to report their income for the current year if it has dropped significantly from last year. In this case, they must also provide one of the following forms:

• 1101 - Declaration of income drop of at least 10% – Father (PDF 117 Kb)
• 1102 - Declaration of income drop of at least 10% – Mother  (PDF 78 Kb)
• 1100 - Declaration of income drop of at least 10% – Spouse (PDF 117 Kb)
